Do I need a referral to see the ENT team?

Some insurance plans require a referral from your primary care provider before you see a specialist, and some do not. We recommend checking with your insurance provider before scheduling. If you are a provider referring a patient to us, you can use our referral form.

What should I bring to my first ENT appointment?

Please bring a photo ID, your insurance card, a list of your current medications, and any relevant records such as prior hearing tests, imaging, or notes from another provider. If your visit is about hearing, balance, or a specific symptom, it helps to jot down when it started and what makes it better or worse. Please arrive 15 minutes early to complete any paperwork.

What insurance plans do you accept?

How do I schedule, cancel, or reschedule an appointment?

To schedule, use our appointment request or call the ENT clinic at 615-341-4725. If you need to cancel or reschedule, please call at least 24 hours in advance whenever possible so we can offer the time to another patient.
